What Relative Fat Mass Measures

Relative Fat Mass, or RFM, is a body fat estimate built from just two numbers: height and waist circumference. It was introduced in a 2018 study published in Scientific Reports, where researchers tested hundreds of anthropometric ratios against DEXA scans and found this simple combination predicted body fat better than BMI.

The idea is that waist size captures abdominal fat, the kind most tied to metabolic risk, while height normalizes that figure to your frame. By dividing height by waist, RFM produces a ratio that rises as fat accumulates around the midsection. Because it skips weight entirely, it sidesteps a known BMI weakness: a muscular person and a heavier person can share the same BMI yet carry very different fat. RFM was validated across a wide range of ages and ethnicities, which makes it a robust quick screen. The RFM Formula Explained

RFM uses a fixed constant, a height-to-waist ratio, and a sex adjustment. With height and waist measured in the same unit, the equations are:

For men:

The only difference is the leading constant, 64 for men and 76 for women, which accounts for women carrying more essential fat. The multiplier of 20 scales the height-to-waist ratio into a percentage. As the waist grows relative to height, the ratio shrinks, the subtracted term gets smaller, and the estimated body fat rises. A man who is 178 cm tall with a 90 cm waist gets a ratio near 1.98, producing an RFM close to 24%. Because both inputs use the same unit, the ratio is dimensionless, so inches and centimeters give the same answer as long as you do not mix them.

Measuring Your Waist for RFM

Since RFM rests on a single circumference, waist technique drives the whole result. The validation study measured the waist at the level of the upper border of the iliac crest, the top of the hip bone, which is slightly lower than the navel for many people. Wrap a flexible tape horizontally around this point, keep it snug without pressing into the skin, and read the value at the end of a normal exhale.

Stand relaxed with feet about shoulder width apart and avoid pulling your stomach in. Take the reading in the morning before eating for the most stable figure, and measure twice, averaging if the two differ by more than 1 cm. Height should be measured without shoes, standing tall against a wall. Because RFM has only two inputs, an error in either one moves the result directly, so a consistent landmark each time matters more for tracking than chasing perfect accuracy on any single day.

RFM Versus BMI

BMI and RFM both aim to flag excess fat, but they measure different things. BMI is weight divided by height squared, so it cannot tell muscle from fat. A lean athlete can register as overweight on BMI, while someone with low muscle and high abdominal fat can pass as normal. RFM ignores weight and reads the waist directly, which is why the original study found it estimated DEXA-measured fat more accurately than BMI, especially in women.

The practical upshot is that RFM is harder to fool with muscle. It targets central adiposity, the fat most linked to cardiovascular and metabolic risk, rather than total mass. Strengths and Limits of RFM

The limits come from the same simplicity. With only height and waist, RFM cannot see fat stored in the hips, thighs, or arms, so it may underread for people who carry fat lower on the body. It is a population-level estimate, so an individual result can differ from a scan by several points. It is also not validated for children or during pregnancy.
